• The applicant’s official identity card.
• A request letter from the forwarding officer is required.
• A certificate issued by the Head of Office must also be provided, following the prescribed format on the Passport Seva Portal.
• A Political Clearance Certificate or PMO Clearance Certificate must be submitted if applicable.
• If the applicant is due to retire within six months from the application date, an official undertaking from the concerned office is required.
• For cancellation or reissue of an existing Diplomatic/Official passport, the applicant is required to submit the original Diplomatic or Official passport, along with the original Surrender/Safe Certificate or cancellation Certificate.
